One operation with two implant shapes, placed under the chest muscle or over it. Volume is set by three measurements rather than by a cup size, and where the nipple sits relative to the crease under the breast decides whether an implant alone is the right operation at all.
Measured, not chosen from a cup size
A silicone implant is placed through a short incision into a pocket behind the breast tissue or beneath the chest muscle. Your base width, upper-pole pinch thickness and existing volume give the range it can be; sizers are then trialled in a bra so the choice is seen rather than read.

No. An implant adds volume but does not raise the nipple or remove loose skin. If your nipple sits at or below the crease beneath your breast, a lift is usually needed as well, either as a combined lift with implant or as a separate procedure.
Size is chosen by measuring your chest width, breast base and the thickness of your tissue, then trying implant sizers in a bra. Your natural anatomy limits how large an implant can safely be, and exceeding it can lead to visible rippling and thinning of tissue over time.
