Where a Beard Transplant Actually Comes From

The follicles are taken from the back of your head, the same donor area a scalp transplant draws on, and placed into facial skin at an angle far flatter than anything on the scalp. Two consequences follow: the design has to be exact, and the grafts you spend here are spent for good.

Your beard grafts come from your scalp. These answers help you plan a beard transplant without shortchanging your hairline.

A beard transplant moves hair follicles from the back of the scalp into the cheeks, jawline, chin or moustache to fill patchy or missing facial hair. It is performed under local anaesthetic, and each graft is placed at the shallow angle facial hair naturally grows at, following a design agreed with you beforehand.

They are taken from the back and sides of your scalp, the same donor area used for a hair transplant. That supply is finite and does not regrow, so every graft placed in the beard is one your hairline cannot use later. If your scalp hair is thinning, this trade-off is discussed before any beard is designed.

The line under the heading tends to stop people: every graft on your jaw is one your hairline will not have. We put it there on purpose. A beard transplant in Turkey (you'll also see it searched for as a beard implant) moves hair from the back of your scalp to your face, and that patch at the back is the same one we'd draw on if your hairline ever needed help. Once it's used, it's used.

None of that is meant to put you off. We see men every week who've spent years with patchy cheeks, a goatee that stops short of the moustache, or a pale scar running through their stubble, and for most of them a beard transplant is one of the more satisfying things we do. We'd just rather you went in with your eyes open, so this page walks through it the way we would at a consultation.

First, look at your hairline

You'll notice the pathway asks for photos of your scalp as well as your face. Most men skip straight past that part. Please don't.

Say you're 27 and your temples have started drifting back a little. A full beard could use around 3,000 grafts. If your hairline keeps going, you may want those same grafts at 40, and they won't be there. Some men weigh that up and still choose the beard, which is completely fair. Others realise they'd rather keep something in reserve. Either way, it's a decision for you to make with the facts, not one to discover later. How many grafts for a beard transplant?

We'd rather you didn't have to guess, so here are the rough figures we work from. A goatee on its own lands somewhere between 500 and 600 grafts. Join it to a moustache and it climbs to 800 or 900. A moustache transplant on its own is a smaller job again. The neat, even stubble a lot of men ask for is roughly 1,500, while a full, classic beard needs about 3,000.

Here's the thing, though. Bare faces are rare in our chair. What turns up in our chair is usually more ordinary. Chin fine. Jaw fine. Then two thin, see-through bits up on the cheeks, or a moustache that gives up a centimetre before it reaches the beard. Fixing that uses a small slice of what a full beard would.

Patchy beard, thin moustache and scars

If we had to name the three things men ask for most, it would be filling the cheeks, joining the moustache to the chin, and thickening the little area just under the lower lip. That's the bulk of beard transplant in Turkey work, and none of it needs huge numbers.

Scars are the other big one. Maybe it's acne. Maybe it's the chin you split falling off a bike as a kid, or a line left by an operation on the jaw years ago. Hair hardly ever grows back through scar tissue, so the mark shows up the moment you stop shaving. Grafts can usually go into a healed scar. Growth is less predictable there, because scars have a poorer blood supply, and now and then a small top-up session is needed. Burn scars can be treated in the same way once the skin has fully settled down.

Drawing the beard: cheek line, neckline, sideburns

Before anything is extracted, the beard is drawn on your face with a marker and you look at it properly. Take your time with this bit.

The cheek line matters more than men expect. Too high, and the beard creeps up towards the eyes. Too low, and the bottom half of the face starts to look heavy. Get the neckline wrong and a strong jaw goes soft, or the other way round. Sideburns are fiddly as well: they need to blend into your scalp hair, not meet it at a seam. Bring along two or three photos of beards you like, on men with faces shaped a bit like yours. And when the marker's on, turn sideways. Mirrors flatter the front.

Why the angle is everything

Scalp hair points upwards and outwards. Beard hair hugs the skin, and it keeps changing its mind about direction: heading down on the cheeks, forward along the jaw, splaying sideways from the centre of the lip. Put a graft in at the wrong tilt and there's no fixing it afterwards. It'll poke out on its own for as long as it grows.

That's why we use single-hair grafts for beards and place each one shallow and by hand. You'll see the page mentions two ways of doing it, with a sapphire blade or with the Choi implanter pen that's also used in DHI. Honestly, the tool matters much less than the experience of the person holding it.

Will a beard transplant look natural?

In most cases, once it's grown in and trimmed, yes. There are two things worth knowing beforehand, though.

The new hairs came from your scalp, so they keep some scalp habits. They can feel a touch softer, lie a little straighter and grow longer than the beard around them. Kept short or medium and trimmed regularly, that difference disappears, and it's exactly what you'll see in the before and after photos on this page. Colour is the other one. If your beard has started going grey and your scalp hasn't, the transplanted hairs will stay darker for a while yet.

Beard transplant recovery: the first month

When you leave the clinic, the treated area is pink and covered in tiny dots of dried blood, one per graft. They usually fall away inside a week. Round the back, where the grafts came from, things settle in about two weeks.

For the first month, three rules matter. You don't wash your face for three days, apart from the gentle cleaning we show you. You don't shave for a month. And you skip sport for a month too, because sweat and friction aren't good for grafts that are still bedding in. Sleeping on your back for the first few nights helps. Most men are back at a desk within a few days; the pink cheeks might earn you a question or two in the first week, and that's about it.

Shaving and trimming afterwards

Once the month is up, treat the new hair the way you treat the rest of your beard. Trim it, shape it with clippers, even shave it off if you fancy a change. It'll come back, because those follicles live in your face now.

That said, we'd suggest leaving the razor alone for the first few months where you can. It's much easier to see how things are filling in when you're not shaving every other day, and your barber will have more to work with when it's time for a proper shape-up.

Beard transplant results month by month

Here's the part nobody enjoys. Two or three weeks after a beard transplant in Turkey, most of the new hairs fall out. It looks like it hasn't worked. It has; the roots are still in the skin, resting. Then come a couple of quiet months. Somewhere around month three or four, fine new hairs start to push through. By six months most men can see a real difference, and at about a year the beard is as full as it's going to get. That's also the earliest point at which a fair before and after photo can be taken.

If you're planning around a wedding or a big birthday, give yourself the full year.

Beard transplant side effects

Expect some redness. Expect your cheeks and lips to puff up a little for a few days, plus the crusts, plus odd numb or tingly patches that wear off over a few weeks. Some men get small spots as the new hairs come through, rather like ingrowns; they tend to sort themselves out. Infections aren't common. Still, if anything goes hot, swollen or really sore, rather than simply pink, photograph it and send it to us the same day.

Not ready for surgery yet?

Twenty-one and fed up with the gaps? Hang on a bit. Beards often keep filling in until the mid-twenties, and plenty of men find the patch that annoyed them at 21 has quietly closed by 25. Some men also try medication on the beard area first, under a doctor's guidance. A transplant really earns its place when the skin in the gaps has nothing left to grow.

Most men make up their minds about a beard transplant in Turkey by scrolling through photos, so here's what we look at when we're judging someone else's work. Direction, before anything else: good beard hair lies flat and runs the way a beard runs, while poor work sticks out and catches the light. The edges come next, especially the cheek line, which should look grown rather than traced. We always check the date, because an "after" taken at four months is a progress shot, not a result. And a long beard can hide a lot, so a short, tidy one tells you more.
