Neither fat technique is universally better. The question is whether a tear-trough hollow sits beneath your bags, because that decides whether the fat is something to get rid of or something to use. Lower blepharoplasty is surgery on the lower eyelids that treats under-eye bags, puffiness and loose skin. Depending on your anatomy, the surgeon removes or repositions the fat that bulges beneath the eye and, if needed, trims a small amount of excess skin to create a smoother transition between the eyelid and cheek.
With age, the thin membrane that holds fat around the eye weakens, allowing fat to bulge forward, while the skin loses elasticity and a hollow can form between the lid and cheek. Lower blepharoplasty addresses these structural changes, which creams and fillers can only partly disguise.
Lower blepharoplasty can reduce shadows caused by bulging fat or a hollow tear trough, which often make dark circles look worse. It cannot change darkness caused by skin pigmentation or visible blood vessels, so your surgeon explains which part of the problem surgery can realistically improve.
Lower-lid surgery on its own is priced after the surgeon has examined how much fat, skin and lid laxity need treating, since repositioning fat or tightening the outer corner changes the work involved. For reference, all four eyelids together are listed at €2,900, and the lower-only figure is confirmed in writing before travel.

When only fat needs to be treated, lower blepharoplasty is often performed through an incision on the inside of the eyelid, known as the transconjunctival approach, which leaves no visible scar. If excess skin also needs removing, a fine incision is made just below the lashes, where it heals discreetly.
If the lower lid is lax, surgery can cause it to pull downward or turn outward. A simple snap-back test and examination of lid tone help the surgeon decide whether additional support, such as tightening the outer corner, is needed to keep the eyelid in a natural position.
Possible complications include prolonged swelling, bruising, dry or watery eyes, asymmetry and, less commonly, a change in the position of the lower lid. Careful assessment of lid support and conservative skin removal help reduce the risk of the eyelid being pulled down.
Expect puffy, bruised lower lids and eyes that feel dry or scratchy for the first several days. Cold compresses, a raised head at night and lubricating drops ease this, while reading and screen time are kept short. Your surgeon reviews the lids in Istanbul before clearing you to fly.
About a week is sensible. Lower-lid swelling peaks in the first days, and if a lash-line incision was used, its stitches are removed before departure. The surgeon also checks that the lower lid is sitting in the correct position, since this is the main concern after lower eyelid surgery.
Contact lenses are usually avoided for at least one to two weeks after lower blepharoplasty, as inserting and removing them can put pressure on healing eyelids. Your surgeon advises when it is safe to resume, and glasses can be worn in the meantime.
Fluid tends to collect in the lower eyelids because of gravity and the thin, delicate tissue in this area. As a result, puffiness after lower blepharoplasty often lasts longer than after upper lid surgery and can take a few months to resolve completely.
Bruising usually fades within two weeks, revealing a smoother lid-to-cheek contour, although the lower lids can stay mildly puffy for longer because fluid collects there. Most surgeons judge the outcome of lower blepharoplasty at around three months, once that residual fluid has cleared. Photographs taken before surgery help you compare the change objectively.
Fat that has been removed or repositioned during lower blepharoplasty rarely returns, so results are long-lasting. The skin and surrounding tissues continue to age naturally, but the eyes typically remain less puffy than they were before surgery. Protecting the delicate skin from sun damage helps maintain the result.
