The back is counted as two treatment zones: the upper back with the bra line, and the lower back. What most people want removed is the discrete fibrous roll at the band rather than the width of the back, which is set by ribcage and frame and which no operation reduces.

The densest fat on the body

Back liposuction removes excess fat from the upper and lower back, including rolls around the bra line and the area just above the waist. It smooths the back's contour so clothing, especially fitted tops and dresses, sits more evenly. The upper and lower back are usually counted as separate zones.
Back liposuction works best for rolls of fat around the bra line or above the waistband that stay despite training, provided the skin over them is still elastic. A quick pinch test shows this: skin that snaps back suggests fat removal will be enough, while slack skin points towards a back lift instead.
If the skin on your back hangs in folds or does not recoil after being pinched, often following major weight loss, removing fat alone may not give a smooth result. In that case, a back lift, which removes excess skin, is usually more effective.

Sitting against a chair back can feel tender at first, but most people manage desk work after about a week, especially with a cushion behind them and the compression garment on. Jobs that involve lifting, bending or twisting usually need a longer break before returning.

Firm swelling across the back often lingers for several weeks, so the smoother outline is best judged between the third and sixth month. By then the skin has usually adapted to the reduced fat, and bra straps and fitted tops no longer create the rolls that were present before surgery.
A smoother back is usually long-lasting, because fat taken from the bra line and lower back does not regenerate. Remaining cells can still expand if you gain weight, which could bring back some fullness, so keeping your weight steady is the best way to protect the result.

Most people never look at their own back. They hear about it from their clothes. A bra that leaves a ridge. A fitted dress that looks fine from the front and odd in a holiday photo taken from behind. A shirt collar that doesn't sit right because of a pad at the base of the neck.
Back liposuction in Turkey deals with exactly these things. We've organised this page around the clothes that tend to give the problem away, because that's how most people describe it to us.
The commonest request by far. Picture a bra strap pressing into the back, with a soft bulge pushing up over it and another under it. Often there's a little extra at the sides too, near the armpit. Under a thin T-shirt you can see it. Some women go up a band size to stop the dig, and the roll stays anyway.
Compared with a soft tummy, this roll is surprisingly solid. Our surgeons often describe it as gristly, with a hard edge where it stops. That makes it harder to remove evenly with ordinary liposuction. This is where we suggest ultrasound more than anywhere else on the body. With VASER liposuction, the fibres are loosened before any suction starts, and the finish is usually more even.
That little pad between the bra cup and the armpit is a separate story. We usually plan it with the arm, not the back. If that's your main concern, our arm liposuction page covers it.
Fitted tops expose more. Across the shoulder blades some women have a smooth, even layer, and in a racerback or a backless dress it makes the whole back look wider than it is. We can thin that layer. We can't move bone, though, so the width across your shoulders stays what it is. If your back is broad but lean, surgery has little to offer.
Our best results here come from backs with fat you can actually grab, the kind that folds into creases when you turn.
At the top of the spine, just below the neck, some people develop a cushion of fat. The popular name is buffalo hump. Collars ride up on it, and jackets hang oddly. Posture makes it look worse, and so does age.
We'll want to know where it came from. Usually the answer is boring: it's just where that person's body puts fat. Now and then it's down to years on steroid tablets, another medicine, or a hormone problem. Those need your own doctor before they need us, so list any regular medication when you write.
When it's ordinary fat, liposuction works well here. The tissue is very fibrous, so this is another place where ultrasound helps. The skin is thick and usually tightens nicely afterwards.
Lower down, the trouble spot sits just above the buttocks. Jeans push it up and over the waistband. Low-rise trousers and bikini bottoms make it hard to miss, so it often comes up before a summer holiday. In men, this lower-back roll is often the main complaint, alongside the love handles.
The lower back is usually treated together with the flanks, since they run into each other. If you're also thinking about the abdomen, you're looking at a full circle around the waist, and 360 liposuction is the page for that.
The skin on the back is the thickest on the body, and it's very good at shrinking back after fat is removed. That's good news. It means loose skin is far less of a problem here than on the arms, thighs or stomach.
It's also why we rarely recommend a back lift. A back lift removes skin through a long scar, often hidden along the bra line. It's €3,500 on our list, the most expensive of our lift operations, and we suggest it least often. It's mainly for people who have lost a very large amount of weight and are left with folds of skin across the back. For almost everyone else, liposuction is enough.
The other thing that makes the back different is that you can't see it. You'll judge the result through photos, other people's comments and how your clothes fit. So we mark the areas on you standing, with a mirror behind you, and show you exactly where we plan to work before surgery.
Don't expect a big number on the surgical notes. The back gives up less fat than a stomach of the same size, partly because it's tough and partly because the safe working layer is thin. The payoff is in outline. The rolls go flat. From shoulder blade down to hip, the silhouette loses its bumps.
Getting dressed is where most people first see it. The strap stops cutting in. A dress zips up without catching. Those small changes are usually what patients mention to us at their check-ups.
Men usually ask about two areas: the lower back roll above the belt and, less often, the upper back. With men we nearly always add ultrasound. Their back fat is tougher still.
Most men who mention their back also have full flanks and some belly. Doing the back alone can make the waist look lopsided from the side, so we tend to plan all three together.
It usually is if there's a definite roll or pad that has sat there for years, even while your weight held steady. It's something you could pinch, and it moves as you turn. The skin on top feels firm. And you're not expecting your ribcage or shoulders to shrink, because they won't.
One exception. A neck pad that appeared fast, or together with a rounder face or bruises that come up too easily, needs your doctor's opinion first. We'll ask about it regardless.
The back is one of the most common places fat is taken from for a Brazilian butt lift. Removing it from the lower back and moving it to the buttocks does two jobs at once: the waist narrows from behind, and the curve below it becomes fuller. Most problems are small and settle. The tough fat on the back makes it slightly more prone to small irregularities, which is one more reason we prefer ultrasound here. Sometimes fluid builds up under the skin and we drain it when you come in for a check. Expect some numb skin over the treated area for a few weeks or months.
The operation is done with you lying on your front. Padding goes under your face, chest and hips, and the anaesthetist rechecks your position regularly. Calf pumps and getting up early keep the risk of clots low, as with any liposuction. Where ultrasound is used, the skin at each entry point is protected from heat.
Because back results show mostly in outline, before and after photos need to be taken from the same angle, with arms in the same position. A raised arm or slight twist changes how the bra line looks more than the surgery does. Look for photos taken from directly behind and from the side.
Reviews from three months onwards give you the clearest idea of the result, once the swelling has settled.
You're asleep under general anaesthetic and lying face down, which needs careful positioning and padding by the anaesthetic team. The area is filled with fluid first, the fibrous fat is softened with ultrasound where agreed, and then removed through small openings placed where a bra strap or waistband will hide them.
Your total volume is reviewed by our anaesthetist before anything is confirmed, especially if the back is being treated alongside other areas.
The garment is already on when you wake. Six weeks or so is the usual stretch. It crosses your shoulder blades and the bra line, which takes some getting used to. A soft front-fastening bra under it, or none at all, is usually the most comfortable.
For roughly the first week, lying flat on your back is sore. Sleep propped up or on your side for a while. Seat backs, including the one on the plane home, press right on the sore area, and a small cushion in the small of your back makes a real difference. Office work is usually fine after a week. Lifting and the gym wait a month or more. The final shape shows somewhere between three and six months.

A quote for the back should never come from a front photo. A price given without a rear view is a guess. Find out as well if the quote is for one zone or two, and if ultrasound is in it. On fibrous back fat, leaving it out to make the price look lower is a false saving.
Ask, too, what happens at the borders, where back meets flank and where it wraps round to the chest. Those transitions are where an uneven result shows first.
