Reasons include discomfort, hardening caused by capsular contracture, implant rupture, concerns about health or long-term safety, changes in body shape or simply no longer wanting implants. Your reason helps the surgeon decide how the capsule should be managed. Some patients simply prefer a more natural, lighter chest after years with implants.
After removal, breasts can look smaller, softer and sometimes deflated or lower, especially if implants were large or in place for many years. This is why some patients choose a breast lift or fat transfer at the same time to create a more intended shape.

An explant is rarely a simple reversal of the original operation. The capsule may need partial or complete removal, bleeding control inside that space takes time, and the stretched breast often needs reshaping. A lift is not automatically included, because not everyone needs one after removal. If your surgeon recommends a lift or fat transfer to improve breast shape, it appears as a separate item in your written plan so you can decide before travel.

Women come to explant surgery from very different places. For some, the implants never felt like part of them. Others had them in their twenties and simply don't want them at fifty. A hardened capsule, a rupture found on a scan, a new sport, a change of heart about size: we hear all of these. Some women arrive with symptoms they believe the implants are causing. Here's the honest version first. Implants stretch the skin over the years, and it won't fully shrink back once they're out. Expect your breasts to be smaller than they were with implants. They'll probably be flatter at the top than before your first operation, too. Some sit a little lower.
Which of those you'll see, and how much, comes down to a handful of things. Larger implants stretch more. So do implants that have been in for many years. Pregnancies since the augmentation make a difference, and so does any big change in weight. Skin quality matters too, which is partly down to age and partly to luck.
The breast goes on changing for months after the operation. In the early weeks swelling makes it look fuller than it will end up. By about a year, the skin has tightened as much as it's going to, and that's when it's fair to judge.
There are three ways the operation can end. The first is removal on its own. The implants come out, the capsule is dealt with as planned, and the breast is left to settle.
The second adds a breast lift in the same operation. A lift takes up the slack skin and moves the nipple higher. You won't gain any size from it. Still, of the three routes, it's the one women seem happiest with afterwards, as opposed to just glad the implants have gone. You'll stay an extra night in Istanbul if a lift is included.
The third uses fat from somewhere you have a little to spare, usually the tummy or thighs, to restore some fullness. It's limited by how much spare fat you have, some of it won't survive, and a second session is sometimes needed. Think of it as a softener for the result rather than a replacement for the implant.
Every implant ends up sitting inside a pocket of scar tissue that your body makes on its own. Surgeons call it the capsule. In most women it stays thin and soft. You wouldn't feel it. At removal, the surgeon has three broad choices. The capsule can be left in place, partly removed, or taken out completely.
Complete removal can be done in one piece with the implant still inside it. That's what "en bloc" means. You'll see it promoted heavily online. Sometimes it's clearly the right call. A suspected lymphoma is one example. Others are a silicone implant that has burst, or a capsule that has turned thick, hard or chalky.
With a thin, healthy capsule it's less clear-cut. Its back wall is often stuck to the ribs and chest muscles. Freeing it adds time and bleeding to the operation, and you'll usually need drains for a few days. There's also a small risk of going through into the chest cavity. A typical email goes something like this. Tired all the time for the last few years. Joints aching. Struggling to focus at work, plus a rash no doctor has been able to explain. By the time she writes to us, the sender has usually come across the term breast implant illness, or BII, already.
We don't doubt what these women are feeling. Many say they feel much better once the implants are out. At the same time, there's no test for the condition and no agreed explanation of how implants might cause it. That means no surgeon can predict whether your own symptoms will improve.
