Which you mostly have decides the operation, and almost everywhere in Europe it also decides the bill. Excess fat is removed with liposuction or VASER through small incisions, and firm glandular tissue is cut out through a small incision at the lower edge of the areola. Many men need both techniques, and the approach is decided after examining the chest.

Adipose tissue is removed by liposuction; glandular tissue behind the areola generally requires surgical excision, and the technique is selected on examination rather than on preference. Whether surgery is appropriate at all depends on an individual clinical assessment.

Most men who contact us about their chest have spent years working round it. Dark T-shirts only. Hoodies in July. Arms folded in every holiday photo, or no holiday photos at all. Some have tried dieting it away, others have added bench press after bench press, and the area behind the nipple hasn't budged. That stubbornness is usually the clue, and it's why gynecomastia surgery in Turkey begins with working out what the swelling is actually made of.
People search for "man boobs surgery" as if it's one problem. It's usually two. Some men carry extra fat across the chest, which surgeons sometimes call pseudogynecomastia. Others have true gynecomastia, a firm, rubbery disc of breast gland sitting right behind the nipple. Plenty have a mix.
You can get a rough idea yourself. Pinch gently behind the nipple. Soft tissue that feels the same as fat elsewhere on your body points one way. A firmer button you can move about between your fingers points the other. It's no substitute for an examination, but it explains why weight loss and gym work help some men and do nothing for others.
For many men it starts in puberty and simply never goes away. Teenage breast swelling is common and often settles by itself within a year or two, so we don't operate on boys who are still developing.
In adults, the list of causes is longer. Weight gain is a big one. Some prescription drugs are known culprits. Steroids are another, and the gym world has added SARMs and prohormones to the list, often sold with little idea of what's actually in the tub. Hormone problems are rarer, as is trouble with the liver or thyroid. Whatever the trigger, it matters if it's still going on. Remove the tissue while the cause is active and it can simply come back, so we look into that first.
Some swellings need a GP before they need us. New ones do. So does a lump that's hard, painful or only on one side. Any fluid leaking from the nipple belongs in that group too. Most of the time the explanation turns out to be harmless. It's still the right order of things, and we'd rather wait a few weeks for your results than skip that step.
There's a grading scale, and where you sit on it decides what the operation involves. Mild cases are a puffy bump around the areola. You'd mainly spot it through a tight T-shirt. A step up, and the fullness spreads across the chest, blurring the line where the pecs should end. At the far end, the extra tissue has started to hang, and there's loose skin to deal with as well.
The first two usually respond well to liposuction, gland removal or both. Where the skin itself needs trimming and the nipple moving up, that's a bigger operation. It isn't one of our standard listed prices, so we'd quote it for your case.
If it's only fat, the surgeon works through a couple of nicks a few millimetres long, usually near the armpit or under the chest. Gland is different. It has to be cut out, through a curved incision at the bottom of the areola, right where the darker skin meets the lighter. That one stays for life. Give it a few months and it tends to blend in where the two skin tones meet. Most people never notice it. Let us know up front about any thick or raised scars you've had before. Darker skin is worth mentioning too.

You'll leave theatre in a compression vest. Nobody will see it under a shirt, which is just as well, because it stays on for weeks. That tight fit isn't for comfort. Don't judge the result in week one. Swelling fills the chest out, and you may wonder whether anything changed at all. Lumpy, hard patches are normal for a while and melt away slowly. Try to sleep on your back. For desk jobs, a week off is usually plenty.
Men who lift ask about this before anything else. You can walk from day one. Light cardio, on a bike or a cross-trainer, usually comes back after about two weeks. Chest work is the one to be patient with. Give bench, push-ups and dips roughly six weeks, because pushing too soon is one of the commonest ways to end up with fluid under the skin.
And if steroids or SARMs had anything to do with it, be honest with yourself about going back on them. They can bring the tissue back, and second operations are fiddlier than first ones.
Gland that has been removed doesn't regrow on its own. What can happen is new tissue forming if the original trigger is still there, whether that's a medicine, steroid use or a hormonal problem. Fat can also come back with weight gain. So we'd like your weight steady before you book. Causes get discussed before any date goes in the diary.
Front-on pictures can look convincing even when the result isn't. When you look at gynecomastia before and after photos, the side view tells you far more. A flat line from collarbone to waist, with nothing pushing out behind the nipple, is what you're after. Half a year after surgery is the earliest point worth comparing before and after shots. Be suspicious of an after shot where he's clearly just come off the bench or is tensing his chest.
