Straight hair goes into the scalp roughly the way it comes out of it. Afro-textured hair does not: the follicle curves beneath the surface, sometimes sharply, and where it goes cannot be read from where it emerges. Every difference on this page follows from that one fact.

Six to eight hours in one day under local anaesthetic, and on paper the same operation as any other. In practice the extraction is slower and less forgiving: each follicle has to be freed along a curve the surgeon cannot see, which usually means a wider punch than a straight-haired case would need and a considerably lower tolerance for haste. An Afro hair transplant is a hair transplant adapted to Afro-textured hair, whose follicles curve beneath the skin as well as above it. Extraction, graft handling and hairline design are all adjusted to that curl, which makes the procedure more specialised than a transplant for straight hair.
Because the root of an Afro follicle is curved, a straight punch can easily cut through it, a problem known as transection. Surgeons adjust punch depth, angle and technique for each graft, which takes more time but protects graft quality and survival.
Yes, when traction alopecia has caused permanent follicle loss. It is important that braids, weaves or tight styles that caused the damage have stopped, otherwise continued tension could affect both native hair and new grafts in the same area. The surgeon also checks whether the follicles in the affected area are truly gone or simply weakened.
