With an unshaven hair transplant (you'll also see it called a no-shave hair transplant) your hair stays at its usual length and quietly covers everything while it heals.
It isn't a different operation. As the section above explains, the extraction and placement are the same; they're simply done through hair that's left at its normal length. What changes is everything around the surgery: how many grafts fit into a day, who qualifies, and what the first few weeks look like from the outside. Three things "unshaven" can mean

The word gets used loosely, which is why the section above advises asking exactly what will be trimmed before asking what it costs.
In a fully unshaven procedure, nothing at all is cut short. Grafts are taken from between the hairs of the donor area and placed between the hairs of the thinning area. This is the most discreet option and the most demanding.
In a hidden donor trim, a narrow band at the back is clipped short for extraction, and the longer hair above it falls down to cover it. Nobody standing behind you would see it unless they lifted your hair.
