Four implants per jaw carrying one fixed arch — and the number is the least interesting part. The two at the back are angled up to 45 degrees, anchoring in the denser bone at the front of the jaw rather than the thin bone at the back.

Two visits, one plan
Two implants go in vertically at the front and two are tilted backwards, into solid bone in front of the sinus above and the nerve below. The tilt also spreads the supports wider than straight implants could — which is what lets an arch sit on four. A fixed provisional goes on the same day; the definitive zirconium arch follows after three to six months.

All-on-4 is a treatment that replaces a full arch of teeth with a fixed bridge supported by four implants. The two front implants are placed upright and the two back implants are tilted, which makes better use of existing bone and often avoids the need for bone grafting.
Tilting the rear implants allows them to engage more available bone while avoiding structures such as the sinus in the upper jaw and the nerve in the lower jaw. This angled placement also lets the bridge extend further back, providing a longer chewing surface with just four implants.
All-on-4 suits people who are missing all or most teeth in one arch, or whose remaining teeth are failing, and who have enough bone for four implants. If bite forces are very strong, the jaw is wide or you grind your teeth, All-on-6 may be recommended instead.
