Position, volume and shape are separate problems. This pathway sorts out which one you have before a single operation is named.
Standing, relaxed photos show where the nipple sits against the crease under the breast. At or below that crease means descent; above it, lost volume may be the whole story.

A key measurement is where the nipple sits relative to the fold beneath the breast while you stand. If the nipple is at or below the fold, the breast has descended and usually needs a lift. If it sits above the fold, lost volume may be the main issue, which an implant can address.
Yes. A breast lift raises and reshapes the breast using your own tissue, and a breast reduction removes excess tissue while lifting what remains. Neither requires an implant, which means there is no future implant exchange or removal to plan for.
