Neither fat technique is universally better. The question is whether a tear-trough hollow sits beneath your bags, because that decides whether the fat is something to get rid of or something to use. 01 · Fat removal

Under-eye bags can come from bulging fat, loose skin or a groove between lid and cheek — and each calls for a different approach.

Lower blepharoplasty is surgery on the lower eyelids that treats under-eye bags, puffiness and loose skin. Depending on your anatomy, the surgeon removes or repositions the fat that bulges beneath the eye and, if needed, trims a small amount of excess skin to create a smoother transition between the eyelid and cheek.
With age, the thin membrane that holds fat around the eye weakens, allowing fat to bulge forward, while the skin loses elasticity and a hollow can form between the lid and cheek. Lower blepharoplasty addresses these structural changes, which creams and fillers can only partly disguise.
Lower blepharoplasty can reduce shadows caused by bulging fat or a hollow tear trough, which often make dark circles look worse. It cannot change darkness caused by skin pigmentation or visible blood vessels, so your surgeon explains which part of the problem surgery can realistically improve.
