Two operations share one name and they are not interchangeable: the upper lid is about the fold of skin above the margin, the lower about the fat beneath the eye. The examination decides which you need — and whether the lid margin itself is the real problem.

Excess skin is removed through the natural upper lid crease, with a small strip of muscle or a little fat where that is what creates the fullness. The lid margin itself is not moved: this operation corrects the fold above it. Where the margin sits low, that is ptosis and needs the levator instead.

Eyelid surgery, or blepharoplasty, removes or repositions excess skin, muscle and fat from the upper and/or lower eyelids. It can reduce hooding of the upper lids and bags under the eyes, creating a fresher, more rested appearance and, in some cases, improving the field of vision.
The choice depends on where the concern lies. Hooded upper lids are treated with upper blepharoplasty, under-eye bags with lower blepharoplasty, and patients with both concerns may benefit from surgery on all four lids in one session. Your surgeon examines both areas before advising.
When heavy upper eyelid skin hangs over the lashes and restricts the upper field of vision, removing the excess can make it easier to see. Eyelid surgery does not correct eyesight itself, and drooping caused by a weak eyelid muscle requires a different procedure called ptosis repair.
